OXYCODONE (Oxycontin)
St. John's wort can increase oxycodone metabolism by inducing cytochrome P450 3A4 (CYP3A4), reducing plasma levels and analgesic activity.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Troxyca ER pairs oxycodone (the active pain reliever) with naltrexone. The piece that matters here is the oxycodone, which your liver clears partly through an enzyme called CYP3A4. St. John's wort speeds that enzyme up. The likely result is that you break down oxycodone faster, so less of it stays in your bloodstream and your pain relief may fade.
In plain terms, your dose could feel weaker than expected, and pain that was controlled might creep back. This is a well-recognized interaction with reasonable evidence behind it. Please don't add, stop, or change anything on your own. Talk with your pharmacist or doctor first, since adjusting either product needs to be done carefully and with guidance.
