Here's the simple version: both Sweet Annie and Avacopan can be hard on your liver. This isn't about one changing the dose of the other. Instead, the worry is that the strain on your liver could add up when you take them together. Avacopan already comes with a known risk of liver problems, so doctors usually check your liver enzymes with blood tests before and during treatment.

I'll be honest, this caution is theoretical and based on weak, anecdotal reports about Sweet Annie affecting the liver. It's not a strong proven interaction. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, nausea, or unusual tiredness. Please check with your pharmacist or doctor before adding Sweet Annie, and don't stop your Avacopan on your own.

Avacopan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the situation with Sweet Annie and Avacopan (Tavneos). Your body clears avacopan using a liver enzyme called CYP3A4, and avacopan is the active drug itself, not a prodrug that needs switching on. So whatever changes that enzyme can change how much avacopan stays in your system.

The tricky part is the evidence is murky. In lab tests, one Sweet Annie compound (artemisinin) seemed to speed up CYP3A4 (which could lower avacopan and weaken its effect), while the whole-plant extract seemed to slow it down (which could raise avacopan and bring more side effects like nausea, headache, or higher liver enzymes). This is only test-tube data, so we honestly can't predict the real-world result. Please loop in your pharmacist or doctor before combining them.

What Is Avacopan Used For?

Brand name: Tavneos

Tavneos (avacopan) is used to treat severe active ANCA-associated vasculitis in adults — specifically the forms known as granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA). These are serious autoimmune conditions in which the immune system attacks small blood vessels, causing damage to the kidneys, lungs, and other organs.

Tavneos is used as an add-on treatment alongside standard therapies, including glucocorticoids (steroid medicines). It is not a replacement for steroids — it works together with them.
