Why this applies to Avapritinib

Avapritinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sweet Annie interacts with.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the situation with Avapritinib (Ayvakit) and Sweet Annie. Your body relies on a liver enzyme called CYP3A4 to clear avapritinib out of your system. Sweet Annie may tug on that enzyme, but the lab data is genuinely mixed. One part of the plant (artemisinin) seems to speed CYP3A4 up, which could lower avapritinib levels and weaken how well it controls your tumor or mast cell disease. Yet the whole-plant extract seems to slow the enzyme down, which could raise levels and worsen side effects like swelling, fatigue, dizziness, low blood counts, or memory and mood changes.

So we honestly can't predict the direction here, and this is only test-tube evidence. Because avapritinib is serious cancer therapy, please don't combine them without checking with your oncology team or pharmacist first.

Sweet Annie (Artemisia annua) is the source of artemisinin, a compound used in prescription antimalarial drugs. While the purified drug is well studied for malaria, the herb itself as a supplement is not proven for the wide range of conditions people use it for, and self-treating malaria with the raw herb is dangerous. About Avapritinib

What Is Avapritinib Used For?

Brand name: Ayvakit

Ayvakit (avapritinib) is used to treat adults with unresectable (cannot be surgically removed) or metastatic (spread to other parts of the body) gastrointestinal stromal tumor (GIST) — but only in patients whose tumor has a specific genetic change called a PDGFRA exon 18 mutation, including the D842V mutation. A test is needed to confirm this mutation before starting treatment.

Ayvakit is also used to treat advanced systemic mastocytosis (AdvSM) — a serious condition where too many mast cells build up in the body — including its subtypes: aggressive systemic mastocytosis (ASM), systemic mastocytosis with an associated blood cancer (SM-AHN), and mast cell leukemia (MCL). It is also approved for indolent systemic mastocytosis (ISM), a less aggressive form. Ayvakit is not recommended for patients with AdvSM or ISM who have very low platelet counts (below 50 × 10⁹/L).
