Here's the simple version. Both Sweet Annie and cyclophosphamide (Cytoxan) have the potential to be hard on your liver. Cyclophosphamide is a chemotherapy drug that your body actually has to activate in the liver, and one of its known risks is liver irritation. The worry here is that stacking Sweet Annie on top could add to that strain. You might see this as fatigue, yellowing of the skin or eyes, dark urine, or belly pain on the right side.

I want to be honest, though: this is mostly theoretical and based on weak, anecdotal reports, not strong studies. Still, since you're already on chemo, I'd play it safe. Please check with your oncologist or pharmacist before adding Sweet Annie, and don't change any cancer medication on your own.

Cyclophosphamide also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's something important about cyclophosphamide (Cytoxan): it's actually a prodrug. Your body has to switch it on using an enzyme called CYP3A4 before it can fight cancer. That makes this combination a little tricky to predict. Sweet Annie may tug on that same enzyme, but it pulls in two directions. One of its compounds (artemisinin) might speed the enzyme up, while the whole-plant extract may slow it down.

If CYP3A4 sped up, you could activate more of the drug and risk more side effects like nausea, low blood counts, or bladder irritation. If it slowed down, you might get less benefit. Honestly, this is based only on lab studies, so we don't truly know what happens in people. With chemo, don't guess. Please run any supplement past your oncologist or pharmacist first.

Sweet Annie (Artemisia annua) is the source of artemisinin, a compound used in prescription antimalarial drugs. While the purified drug is well studied for malaria, the herb itself as a supplement is not proven for the wide range of conditions people use it for, and self-treating malaria with the raw herb is dangerous. About Cyclophosphamide

What Is Cyclophosphamide Used For?

Brand names: Cytoxan, Neosar

Cyclophosphamide is used to treat many types of cancer in adults and children, including malignant lymphomas (such as Hodgkin's disease and Burkitt's lymphoma), multiple myeloma, several forms of leukemia, breast cancer, ovarian cancer, neuroblastoma, retinoblastoma, and advanced mycosis fungoides (a type of skin lymphoma). It is often given alongside other chemotherapy drugs rather than on its own.

Oral and injectable powder formulations (but not the Cyclophosphamide Injection solution concentrate) are also approved for minimal change nephrotic syndrome — a kidney condition — in children who have not responded well to, or cannot tolerate, steroid therapy. The injectable solution concentrate is approved for adult patients only due to its alcohol content, and is not approved for use in children.
