Why this applies to Darunavir, Cobicistat
Darunavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sweet Annie interacts with.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing about Prezcobix (darunavir with cobicistat): both pieces lean heavily on the liver enzyme CYP3A4. Cobicistat is actually in there on purpose to block that enzyme and keep your darunavir levels high enough to fight HIV. So anything that messes with CYP3A4 could nudge your darunavir levels in either direction.
Sweet Annie is a wild card here. Lab studies are mixed, with one signal saying its artemisinin may speed CYP3A4 up while the whole-plant extract seems to slow it down. If it sped things up, darunavir could drop too low and let the virus gain ground, which risks resistance. The honest answer is we just don't have human data to say. Because keeping HIV controlled is too important to gamble on, please run this past your pharmacist or doctor before adding Sweet Annie.

What Is Darunavir and Cobicistat Used For?
Brand name: Prezcobix
Prezcobix and Prezcobix Ped are used to treat HIV-1 (human immunodeficiency virus type 1) infection. They are always used together with other antiretroviral (anti-HIV) medicines — never alone. Both products are approved for people who are new to HIV treatment (treatment-naïve) and for those who have taken HIV medicines before (treatment-experienced), as long as certain darunavir resistance mutations are not present.
Prezcobix tablets are approved for adults and pediatric patients weighing at least 40 kg. Prezcobix Ped tablets for oral suspension extend coverage to pediatric patients 3 years of age and older weighing at least 15 kg.
