Dofetilide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sweet Annie interacts with.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the honest picture with Sweet Annie and dofetilide (Tikosyn). Dofetilide is a heart rhythm drug with a very narrow safety window, meaning small shifts in how much is in your body can matter a lot. Part of it is handled by a liver enzyme called CYP3A4, and Sweet Annie tugs on that enzyme in lab studies. The tricky part is that one constituent (artemisinin) seemed to speed the enzyme up, while the whole extract slowed it down, so the net direction in real people isn't clear.

Because dofetilide can affect heart rhythm and, at higher levels, raise the risk of a dangerous fast heartbeat, I wouldn't gamble on an unpredictable herb here. This is only test-tube evidence, not proof, but given how sensitive dofetilide is, please check with your doctor or pharmacist before combining them.

About Dofetilide

What Is Dofetilide Used For?

Brand name: Tikosyn

Dofetilide capsules are used to convert atrial fibrillation (AFib) and atrial flutter (abnormal, rapid heart rhythms) back to normal sinus rhythm, and to help keep those rhythms from coming back. This applies to patients who have had AFib or atrial flutter lasting more than one week and who have already been converted to a normal rhythm.

Because dofetilide can cause serious, life-threatening heart rhythm problems, it is reserved for patients whose AFib or atrial flutter causes significant symptoms. It has not been shown to be effective in paroxysmal atrial fibrillation (AFib that starts and stops on its own). Tikosyn is an FDA-approved brand of dofetilide capsules with these same indications.
