Here's the concern with pairing Sweet Annie and Fruquintinib (Fruzaqla). This isn't about one changing the blood level of the other. Instead, both can be tough on your liver, so the worry is that stress may add up. Fruquintinib, a cancer medication that blocks tumor blood vessel growth, can already raise liver enzymes and cause liver problems in some people, and Sweet Annie has its own reports of liver irritation.

To be fair, this is mostly a theoretical concern based on weak, anecdotal evidence, not solid studies. Still, since your cancer team likely checks your liver labs regularly, adding an herb that could muddy those results isn't ideal. Please don't start or stop anything on your own. Just loop in your oncologist or pharmacist first so they can weigh in.

Fruquintinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the situation with Sweet Annie and Fruquintinib (Fruzaqla), the oral cancer drug used for certain advanced colorectal cancers. Fruquintinib is cleared from your body partly by a liver enzyme called CYP3A4, and Sweet Annie may nudge that enzyme in either direction. The lab data are honestly mixed: one constituent (artemisinin) seems to speed the enzyme up, while the whole-plant extract seems to slow it down. So we can't say for sure which way it would tip Fruquintinib.

If Sweet Annie sped clearance up, your drug level could drop and work less well against the cancer. If it slowed clearance, levels could climb and worsen side effects like high blood pressure, fatigue, mouth sores, or hand-foot skin reactions. This is only test-tube evidence, so please loop in your oncology team before combining them.

What Is Fruquintinib Used For?

Brand name: Fruzaqla

Fruzaqla (fruquintinib) is used to treat adults with metastatic colorectal cancer (mCRC) — colorectal cancer that has spread to other parts of the body. It is specifically for patients who have already been treated with several standard chemotherapy regimens and other targeted therapies.

Fruzaqla is intended for patients who have previously received fluoropyrimidine-, oxaliplatin-, and irinotecan-based chemotherapy, an anti-VEGF therapy, and — if their tumor is RAS wild-type and it is medically appropriate — an anti-EGFR therapy.
