Here's the gist with nilotinib (Tasigna) and Sweet Annie. This isn't about one speeding up or slowing down the other. Instead, the worry is that both can be hard on the liver, so using them together may add up. Nilotinib already carries a warning for liver strain, and it can raise liver enzymes or bilirubin. Sweet Annie has been linked to possible liver irritation too.

Honestly, this concern is theoretical and based mostly on scattered reports, not strong studies, so I don't want to alarm you. Still, since your nilotinib needs careful liver monitoring, watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your oncologist or pharmacist before adding this herb.

nilotinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the situation with Sweet Annie and your nilotinib (Tasigna). Nilotinib is an active drug (not a prodrug) that your body clears using a liver enzyme called CYP3A4. Sweet Annie may tug on that enzyme, but honestly, the lab data pull in two different directions. One compound in it (artemisinin) seems to speed CYP3A4 up, while the whole-plant extract seems to slow it down, so the net effect in a real person is unclear.

If it sped up clearance, your nilotinib level could drop and the leukemia treatment might work less well. If it slowed clearance, levels could climb and raise the risk of side effects like a dangerous heart rhythm change (QT prolongation), nausea, or low blood counts. Since nilotinib is a serious cancer medicine and this is only test-tube evidence, please check with your oncologist or pharmacist before combining them.

What Is Nilotinib Used For?

Brand name: Tasigna

  • Nilotinib is used to treat certain types of chronic myeloid leukemia (CML; a type of cancer of the white blood cells) who have recently found to have this condition in adults and children 1 year of age and older. It is also used to treat certain types of CML in adults whose disease could not be treated successfully with imatinib (Gleevec) or adults who cannot take imatinib because of side effects. Nilotinib is also used to treat certain types of CML in children 1 year of age or older whose disease could not be treated successfully with other tyrosine kinase inhibitor therapies or who cannot take these medications because of side effects. Nilotinib is in a class of medications called kinase inhibitors. It works by blocking the action of the abnormal protein that signals cancer cells to multiply. This helps to stop or slow the spread of cancer cells.
