Akeega combines niraparib and abiraterone, a two-in-one pill used for certain advanced prostate cancers. One thing your care team already watches closely with this medicine is your liver, since abiraterone in particular can raise liver enzymes and, less often, cause real liver strain. That's why you get regular blood tests while on it.

Here's the concern with Sweet Annie: this herb has been linked to possible liver irritation on its own. Stacking it on top of a medication that already taxes the liver could, in theory, add up to more stress on that organ. I'll be honest, this is based on weak, anecdotal evidence, not strong studies, so don't panic. Still, since your liver is being monitored anyway, please run any Sweet Annie use past your oncologist or pharmacist first.

Niraparib, Abiraterone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Akeega combines two prostate cancer medicines, and the abiraterone part relies on the liver enzyme CYP3A4 to be cleared. Here's the honest picture: Sweet Annie's effect on that enzyme is confusing. In lab studies, the artemisinin in it may speed the enzyme up, while whole plant extract seemed to slow it down. Because these results conflict and come only from test-tube work, we can't reliably say which way things would tip in a real person.

Since this is serious cancer therapy, I wouldn't gamble on it. If Sweet Annie sped up clearance, abiraterone might work less well; if it slowed clearance, you could see more fatigue, high blood pressure, or liver strain. Please check with your oncology team or pharmacist before combining these.
