Here's the simple version. Both your norgestrel birth control pill and Sweet Annie are processed by your liver, and there's a concern that Sweet Annie may stress the liver in some people. The worry is that putting the two together could add up and put a little extra strain on this organ. This isn't about your pill working better or worse, it's mostly about liver health.

I want to be honest about how thin the evidence is here. This is a theoretical concern based mainly on scattered, anecdotal reports, not solid studies. Still, since liver issues can be sneaky, watch for things like yellowing of the skin or eyes, dark urine, belly pain, or unusual tiredness. Before mixing them, please check with your pharmacist or doctor.

Norgestrel also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the practical worry with Sweet Annie and your norgestrel mini-pill. Norgestrel is a hormone your body clears using a liver enzyme called CYP3A4. Sweet Annie's artemisinin has been shown, in lab dishes only, to speed that enzyme up. If that happened in real life, your body might break down the progestin faster, leaving less of it to do its job. With a progestin-only pill, lower levels can mean weaker protection against pregnancy and possibly more breakthrough spotting or irregular bleeding.

I want to be honest though: this is based on test-tube data, not human studies, and the same plant also seemed to block the enzyme in other tests, so the real-world effect is genuinely unclear. Since your birth control is at stake, please run this by your pharmacist or doctor before combining them.

What Is Progestin-Only (norethindrone) Oral Contraceptives Used For?

Brand names: Ovrette, Orval 21

Norethindrone progestin-only pills — sometimes called the "mini-pill" — are used solely to prevent pregnancy. They do not contain estrogen, which makes them an option for women who cannot or prefer not to use estrogen-containing contraceptives.

When taken perfectly (same time every day, no missed pills), fewer than 1 in 100 women will become pregnant in the first year. With typical use, that number rises to about 9 in 100 — mostly because of late or skipped pills. Consistency is everything with this method.
