Here's the concern with pairing Sweet Annie and Repotrectinib (Augtyro), your targeted cancer therapy for certain ROS1 and NTRK-positive tumors. This isn't about one changing how the other is processed. Instead, both can be tough on the liver, so taking them together may add up and put more stress on that organ.

Repotrectinib already carries a known risk of raising liver enzymes and, less often, causing more serious liver injury. Sweet Annie has also been linked to possible liver harm, though honestly that evidence is thin and mostly anecdotal. Still, I'd play it safe. Watch for yellowing of the skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your oncologist or pharmacist before adding this herb.

Repotrectinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the short version on pairing Sweet Annie with Augtyro (repotrectinib), your cancer medicine for certain ROS1 and NTRK-positive tumors. Repotrectinib is broken down by a liver enzyme called CYP3A4, and Sweet Annie may nudge that enzyme in either direction. The tricky part is that the evidence here is weak and conflicting: in lab dishes, artemisinin (Sweet Annie's key compound) seems to speed up CYP3A4, while the whole-plant extract seems to slow it down. Nobody has confirmed what actually happens in real people.

If it sped things up, your repotrectinib levels could drop and it might not fight the cancer as well. If it slowed things down, levels could climb, raising the chance of side effects like dizziness, numbness or tingling, and trouble with balance or thinking. Because this is a serious medication, please don't add Sweet Annie without checking with your oncologist or pharmacist first.
