Here's the simple version. Ripretinib (Qinlock) is a cancer medicine that your liver has to process, and it can sometimes raise your liver enzymes or stress the liver on its own. Sweet Annie, taken as a concentrated herbal supplement, has also been linked to possible liver irritation in some reports. Put the two together and the worry is that they could gang up and be harder on your liver than either one alone.

I'll be honest, this concern is theoretical and based on weak, anecdotal evidence, so it isn't a sure thing. Still, with a cancer treatment like this, I'd play it safe. Watch for yellowing of the skin or eyes, dark urine, belly pain, or unusual tiredness, and please run any Sweet Annie use past your oncologist or pharmacist first.

Ripretinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the situation with Sweet Annie and your ripretinib (Qinlock). Your liver uses an enzyme called CYP3A4 to break ripretinib down, and Sweet Annie may bump into that process. The tricky part is that the research is mixed. In lab studies, the artemisinin in Sweet Annie sped this enzyme up, while whole-plant extract seemed to slow it down. So we honestly can't say for sure which way it would push your ripretinib levels.

If the enzyme speeds up, ripretinib could clear faster and work less well against your tumor. If it slows down, the drug could build up and bring more side effects like fatigue, hair thinning, muscle aches, or high blood pressure. Since this is a cancer medicine, I'd skip the guesswork. Please run it by your oncologist or pharmacist before adding Sweet Annie.
