Here's the concern with pairing Sweet Annie and Sparsentan (Filspari). This isn't about one changing the other's blood levels. Instead, both can be tough on the liver, so taking them together might add up and put extra strain on it. Sparsentan already carries a known risk of liver injury, which is why people on it get regular blood tests to watch their liver enzymes. Sweet Annie has been linked, in a less proven way, to liver trouble too.

To be honest, this warning rests mostly on theory and scattered reports, not solid studies, so I don't want to alarm you. Still, since Sparsentan needs careful liver monitoring, I'd check with your doctor or pharmacist before adding Sweet Annie. Watch for yellowing skin, dark urine, belly pain, or unusual tiredness.

Sparsentan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the situation with Sweet Annie and Sparsentan (Filspari). Your body relies on a liver enzyme called CYP3A4 to break down sparsentan, and Sweet Annie may tinker with that enzyme. The tricky part is the research points in two directions: one compound (artemisinin) might speed the enzyme up, while the whole-plant extract seems to slow it down. So we genuinely can't predict which way your sparsentan levels would shift.

If levels rose, you might see more of sparsentan's effects, like low blood pressure, swelling, or dizziness. If they fell, the medicine could work less well for your kidney condition. Importantly, this is based only on lab studies, not real patients, so it's far from certain. Since sparsentan already needs careful monitoring, please check with your pharmacist or doctor before adding Sweet Annie.
