Prevpac is a combination pack (lansoprazole, amoxicillin, and clarithromycin) used as a short course to clear an H. pylori stomach infection. The concern here isn't about Syrian Rue changing how these medicines work in your body. Instead, it's about a shared burden on your liver. Both clarithromycin and large amounts of Syrian Rue seeds can each stress the liver on their own, so combining them could, in theory, add up.

To be honest, this is a theoretical concern based on how things work, not strong human studies. The reported liver problems came from people swallowing huge amounts of seeds (up to 100 grams). Still, watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please check with your pharmacist or doctor before mixing the two.

Theoretically, Syrian rue might increase the levels and clinical effects of drugs metabolized by CYP3A4.
Syrian rue contains beta-carboline alkaloids, including harmine, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids competitively inhibit CYP3A4 enzymes.

Prevpac is a combo pack used to clear out an H. pylori stomach infection, and one of its ingredients, clarithromycin, is mainly handled by a liver enzyme called CYP3A4. Lab studies suggest the harmala alkaloids in Syrian Rue can slow that same enzyme down. In theory, that could let clarithromycin build up a bit higher than usual.

If that happened, you might notice more of clarithromycin's usual side effects, like nausea, an upset stomach, a strange taste in your mouth, or rarely heart rhythm changes. Keep in mind this is theoretical, based only on test-tube data, not real people, so it may not amount to much. Still, given Syrian Rue's own toxicity risks, please run it by your pharmacist or doctor before combining the two.

What is Syrian Rue?

Syrian Rue (Peganum harmala) is a traditional plant whose seeds contain potent compounds called harmala alkaloids, including MAO-inhibiting harmine and harmaline. There is little reliable human evidence that it safely treats any condition, and it carries real risks of toxicity, so it is not recommended for self-treatment. Talk to a pharmacist or doctor before considering it.
