Lorlatinib falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here the concern runs the other direction than usual. Taking your lorlatinib (Lorbrena) cancer pill at the same time as a thyroid extract supplement isn't expected to change how lorlatinib works. Instead, the worry is that lorlatinib, like food and other pills, may get in the way of the thyroid hormone being absorbed, so the thyroid product could end up weaker and less predictable.

This is rated a minor interaction, and it's really about timing more than danger. Honestly though, the bigger issue is that over-the-counter thyroid extract isn't standardized, and you don't want anything competing with a serious cancer medicine like lorlatinib. Please loop in your oncology pharmacist or doctor before combining them so they can guide spacing and safety.

What Is Lorlatinib Used For?

Brand name: Lorbrena

Lorbrena (lorlatinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — meaning lung cancer that has spread — when lab testing confirms the tumor is ALK-positive (has a specific gene change that drives cancer growth). Patients must be tested with an FDA-approved diagnostic test before starting treatment.

Lorbrena has been studied in patients who have not yet received treatment for advanced disease, as well as in patients whose cancer progressed on other ALK-targeting medicines.
