CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Lorlatinib
Lorlatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Atractylodes interacts with.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Lorbrena (lorlatinib). Your body clears this cancer drug largely through a liver enzyme called CYP3A4. In animal studies, high doses of atractylodes appeared to slow that same enzyme down. If that happened in people, lorlatinib could build up a bit more than expected, which in theory might mean stronger side effects like high cholesterol or triglycerides, swelling, mood or memory changes, or fatigue.
The important part: this is only a theoretical, lab-and-animal concern, and it has not been shown in humans. Lorlatinib is not a prodrug, so it doesn't get flipped around. Still, because this is serious cancer treatment, please run any atractylodes-containing herbal product by your oncologist or pharmacist before starting it.

Lorbrena (lorlatinib) is used to treat adults with metastatic non-small cell lung cancer (NSCLC) — meaning lung cancer that has spread — when lab testing confirms the tumor is ALK-positive (has a specific gene change that drives cancer growth). Patients must be tested with an FDA-approved diagnostic test before starting treatment.
Lorbrena has been studied in patients who have not yet received treatment for advanced disease, as well as in patients whose cancer progressed on other ALK-targeting medicines.
