Thyroid Extract and Phenylephrine

Here is the interaction we found between these two, with the evidence behind it.

Minor

Oral Drugs

Why this applies to Phenylephrine

Phenylephrine falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.
Severity Minor Likelihood Likely Evidence A

Well-controlled human studies have demonstrated the likely existence of this interaction.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the practical takeaway when you pair thyroid extract with phenylephrine (the decongestant in products like Sudafed PE). The interaction on file is about absorption timing, mostly tied to taking thyroid hormone too close to food, minerals, or other oral medicines. It's rated minor, so this isn't a dangerous combination, but it can blunt how well your thyroid supplement gets absorbed if you take everything at once.

One thing worth knowing on the symptom side: both thyroid hormone and phenylephrine can speed up your heart and make you feel jittery or anxious, so together you may notice a stronger racing-heart or wired feeling. To keep absorption steady, space your thyroid product away from other pills. Since over-the-counter thyroid extract isn't standardized, please run this by your pharmacist or doctor.

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

From our pharmacist’s overview on the Phenylephrine page

“If you take phenylephrine (Sudafed PE, Biorphen) for congestion, treat this list as one that needs real attention, since six of its 81 entries carry a major rating. Phenylephrine tightens blood vessels to clear your nose, and more than half of the interactions here come from stimulant ingredients, ephedra above all, that can push blood…”

Thyroid extract supplements are made from dried animal thyroid gland and may contain real thyroid hormones (T3 and T4), but the amounts in over-the-counter products are not standardized and can be unpredictable. Self-treating a thyroid problem with these supplements can be risky, so always work with a doctor or pharmacist and use prescription thyroid medicine when treatment is needed.
