Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here the concern runs the other direction from what you might expect. It isn't that pralsetinib (Gavreto) changes your thyroid extract. Rather, the issue is that taking another oral pill alongside the supplement, like pralsetinib, can sit in your stomach at the same time and interfere with how well the thyroid hormones in the extract are absorbed. This is a well-documented, but generally minor, timing issue.

The simple fix is spacing. Many people take thyroid hormone on an empty stomach and wait before other medicines. Since you're managing a serious cancer treatment, please don't rearrange anything on your own. Let your oncology team and pharmacist know you're using a thyroid extract, since unregulated products can also cloud your thyroid lab results.

About Pralsetinib

What Is Pralsetinib Used For?

Brand name: Gavreto

Gavreto (pralsetinib) is used to treat adults with metastatic RET fusion-positive non-small cell lung cancer (NSCLC) — a type of lung cancer that has spread and carries a specific genetic alteration detected by an FDA-approved test.

Gavreto is also approved for adults and children aged 12 and older with advanced or metastatic RET fusion-positive thyroid cancer who need systemic (whole-body) treatment and whose cancer no longer responds to radioactive iodine (when radioactive iodine is an appropriate option). This thyroid cancer use is under accelerated approval, meaning continued approval depends on results from confirmatory clinical trials.
