Garcinia and Pralsetinib

Here is the interaction we found between these two, with the evidence behind it.

HEPATOTOXIC DRUGS

Why this applies to Pralsetinib

Pralsetinib falls under HEPATOTOXIC DRUGS — the group Garcinia interacts with.

Theoretically, concomitant use with other potentially hepatotoxic drugs might increase the risk of developing liver damage.
There have been reports of acute hepatitis with elevated liver enzymes associated with garcinia, when taken alone or in combination with other ingredients. Case reports collected from the Drug Induced Liver Injury Network suggest this risk may be greater in people who carry the HLA B*35:01 allele.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the concern with pairing Garcinia and pralsetinib (Gavreto). This isn't about one speeding up or slowing down the other. Instead, it's about a shared side effect on your liver. Pralsetinib is already known to raise liver enzymes and, in some people, cause liver injury. Garcinia has its own rare reports of liver inflammation. Stacking the two could, in theory, add to that strain.

I'll be honest, this is a theoretical caution based on how each acts, not strong proof. But since your cancer team is likely watching your liver labs already, it's worth mentioning Garcinia before you start it. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and call your doctor if those show up.

From our pharmacist’s overview on the Pralsetinib page

“With pralsetinib (Gavreto), a targeted therapy for RET-driven lung and thyroid cancers, I would treat this list as required reading rather than background noise. Most of the interactions change how much of the drug the body actually gets: grapefruit can push its levels higher, while St.”

About Garcinia

What is Garcinia?

Garcinia
  • Weight loss
  • Appetite suppression
  • Curbing sugar and carb cravings
  • Supporting metabolism
  • Traditional cooking and souring agent

Garcinia is a tropical fruit whose rind contains hydroxycitric acid (HCA), widely marketed for weight loss and appetite control. The scientific evidence is weak and mixed, with most studies showing only small or no meaningful weight effects, and rare reports of liver problems raise safety concerns. Talk to a pharmacist or doctor before trying it, especially if you have liver issues, are pregnant, or are breastfeeding.

About Pralsetinib

What Is Pralsetinib Used For?

Brand name: Gavreto

Gavreto (pralsetinib) is used to treat adults with metastatic RET fusion-positive non-small cell lung cancer (NSCLC) — a type of lung cancer that has spread and carries a specific genetic alteration detected by an FDA-approved test.

Gavreto is also approved for adults and children aged 12 and older with advanced or metastatic RET fusion-positive thyroid cancer who need systemic (whole-body) treatment and whose cancer no longer responds to radioactive iodine (when radioactive iodine is an appropriate option). This thyroid cancer use is under accelerated approval, meaning continued approval depends on results from confirmatory clinical trials.
