Pralsetinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP3A4 and reduce metabolism of CYP3A4 substrates.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.

Here's the situation with goji and your pralsetinib (Gavreto). Your body relies on a liver enzyme called CYP3A4 to break down pralsetinib and keep its level in a healthy range. Lab studies hint that goji berry juice may slow this enzyme down. If that happened in real life, pralsetinib could clear more slowly and build up a bit, which might mean more of its usual side effects like high blood pressure, swelling, tiredness, mouth sores, or changes in your blood counts.

The honest part: this is only a test-tube finding, and it has never actually been reported in people. Eating goji berries as a regular food is unlikely to matter. Concentrated goji supplements or large amounts of juice are the bigger question mark. Since pralsetinib is a serious cancer medicine, please check with your oncology team or pharmacist before adding any goji product.

What Is Pralsetinib Used For?

Brand name: Gavreto

Gavreto (pralsetinib) is used to treat adults with metastatic RET fusion-positive non-small cell lung cancer (NSCLC) — a type of lung cancer that has spread and carries a specific genetic alteration detected by an FDA-approved test.

Gavreto is also approved for adults and children aged 12 and older with advanced or metastatic RET fusion-positive thyroid cancer who need systemic (whole-body) treatment and whose cancer no longer responds to radioactive iodine (when radioactive iodine is an appropriate option). This thyroid cancer use is under accelerated approval, meaning continued approval depends on results from confirmatory clinical trials.
