Brexpiprazole falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Tinospora Cordifolia interacts with.

Theoretically, Tinospora cordifolia might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that Tinospora cordifolia extract inhibits CYP2D6 at high concentrations. However, this interaction has not been reported in humans.

Here's the gist with Brexpiprazole (Rexulti) and Giloy. Your body relies partly on a liver enzyme called CYP2D6 to clear brexpiprazole out of your system. In lab studies, Tinospora cordifolia can slow that same enzyme down (at fairly high concentrations). If that happened in a person, brexpiprazole could build up a bit, which may mean more of its usual side effects, like drowsiness, restlessness or an urge to keep moving, dizziness, or weight gain.

The reassuring part: this is only a theoretical concern based on test-tube data, and it has not actually been seen in people. Brexpiprazole is not a prodrug, so the direction here would be stronger, not weaker, effects. Food-level amounts are unlikely to matter, but concentrated supplements are a fair question. Please run it by your pharmacist or doctor before combining them.

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
