Cilostazol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Tinospora Cordifolia interacts with.

Theoretically, Tinospora cordifolia might increase levels of drugs metabolized by CYP2C19.
In vitro research shows that Tinospora cordifolia extract inhibits CYP2C19 at high concentrations. However, this interaction has not been reported in humans.

Here's the gist for cilostazol (Pletal). Your body relies on a liver enzyme called CYP2C19 to help break this medicine down. In lab dish studies, Tinospora cordifolia (Giloy/Guduchi) appears to slow that enzyme when used in high concentrations. In theory, that could let cilostazol build up a bit more than usual, which might nudge its blood-thinning and blood-vessel-widening effects upward. That could mean more headache, dizziness, a pounding or racing heartbeat, loose stools, or easier bruising.

I want to keep this in perspective. This is only test-tube evidence, and it hasn't actually been seen in real people taking both together. So I wouldn't panic. Still, since Giloy has also been tied to liver injury, it's smart to check with your pharmacist or doctor before pairing the two rather than starting or stopping anything on your own.
