Tinospora Cordifolia and Clozapine

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Tinospora Cordifolia interacts with whole categories of medication. Clozapine falls into 2 of those categories, so it can interact with Tinospora Cordifolia in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

1 of 2

Why this applies to Clozapine

Clozapine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Tinospora Cordifolia interacts with.

Theoretically, Tinospora cordifolia might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that Tinospora cordifolia extract inhibits CYP2D6 at high concentrations. However, this interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for clozapine: your body partly relies on a liver enzyme called CYP2D6 to clear it out. In lab-dish studies only, Tinospora cordifolia (Giloy) appears to slow that enzyme down when used at high concentrations. If that were to happen in a real person, clozapine could build up a bit more than usual, which might mean stronger drowsiness, dizziness, low blood pressure, more drooling, constipation, or a faster heartbeat.

I want to be honest about the evidence, though: this is only a test-tube finding and hasn't been seen in actual patients. Still, clozapine is a drug we watch carefully, and Giloy itself has been linked to liver injury. Before combining them, please check in with your prescriber or pharmacist so they can keep an eye on your dose and levels.

Theoretically, Tinospora cordifolia might increase levels of drugs metabolized by CYP1A2.
In vitro research shows that Tinospora cordifolia extract inhibits CYP1A2 at high concentrations. However, this interaction has not been reported in humans.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing with clozapine: your liver leans heavily on an enzyme called CYP1A2 to clear it from your body. In lab dish studies, Tinospora cordifolia (Giloy) appears to slow that enzyme down a bit. If that happened in real life, clozapine could build up higher than expected, which might mean more drowsiness, dizziness, drooling, a faster heartbeat, low blood pressure, or in rare cases seizures.

Now, I want to be honest with you. This is only seen in test tubes at high concentrations, never confirmed in people. Still, clozapine is a tricky, closely-monitored drug, so even small shifts deserve respect. On top of that, both clozapine and Giloy have been linked to liver issues. Please loop in your prescriber or pharmacist before combining them.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

From our pharmacist’s overview on the Clozapine page

“Clozapine (Clozaril, Versacloz) is usually the last-resort antipsychotic for schizophrenia, prescribed under close monitoring because small shifts in its blood level matter, so the 212 supplement interactions on file here deserve genuine care rather than a quick skim. Six are rated major.”

Tinospora cordifolia, known as Guduchi or Giloy in Ayurvedic medicine, is a climbing plant traditionally used to support immunity and treat fevers. About Clozapine

What Is Clozapine Used For?

Brand names: Clozaril, Versacloz

Clozapine is used for treatment-resistant schizophrenia. That means severely ill people with schizophrenia who have not responded well enough to standard antipsychotic treatment. Because of its risks, it is meant only for those people.

It is also used to reduce the risk of repeated suicidal behavior in people with schizophrenia or schizoaffective disorder who are judged to be at ongoing risk. This is supported for Clozaril tablets, Versacloz oral suspension, Clozapine orally disintegrating tablets and Clozapine tablets.
