ORAL DRUGS
Why this applies to Metronidazole, Tetracyline, Bismuth Subcitrate Potassium
Metronidazole, Tetracyline, Bismuth Subcitrate Potassium falls under ORAL DRUGS — the group Tragacanth interacts with.
Tragacanth contains hydrocolloidal fiber which can decrease, increase, or not affect oral drug absorption. To avoid changes in absorption, take tragacanth 30-60 minutes after oral medications.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: tragacanth is a plant gum that swells into a thick, slippery gel when it hits fluid in your gut. The concern with Pylera (which packs metronidazole, tetracycline, and bismuth into one capsule) is that this gel could trap some of the medicine and keep your body from absorbing all of it. If that happens, the antibiotics may not reach the levels needed to fully clear the H. pylori infection, and an ulcer that isn't fully treated can come back.
This is only a theory based on how the gum behaves, not something proven in people, so don't panic. To stay on the safe side, simply separate them: take your Pylera first, then wait at least 30 to 60 minutes before any tragacanth. When in doubt, your pharmacist can help you map out the timing.

About Bismuth, Metronidazole, and Tetracycline
What Is Bismuth, Metronidazole, and Tetracycline Used For?
Brand name: Pylera
This combination capsule treats Helicobacter pylori (H. pylori) infection in people who have a duodenal ulcer (an ulcer in the first part of the small intestine) or have had one within the past 5 years. It is always used together with omeprazole.
Clearing H. pylori has been shown to lower the chance that a duodenal ulcer comes back. The same use is listed for the generic bismuth subcitrate potassium, metronidazole and tetracycline hydrochloride capsules and for Pylera. It should only be used for infections proven or strongly suspected to be caused by bacteria.
