Why this applies to Alvimopan

Alvimopan falls under P-GLYCOPROTEIN SUBSTRATES — the group White Pepper interacts with.

Theoretically, white pepper might increase levels of p-glycoprotein substrates. The piperine constituent of white pepper seems to inhibit p-glycoprotein in vitro.
Some drugs metabolized by p-glycoprotein include some chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), digoxin, corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Here's the gist for Entereg (alvimopan). This medicine relies on a tiny pump in your gut wall called P-glycoprotein, which normally helps move it back out. The piperine in white pepper has been shown, in test-tube studies only, to slow that pump down. If the pump works less, a bit more alvimopan could stay in your system, which in theory could nudge its effects up slightly.

Honestly, this is a theoretical concern based on lab work, not real patient cases, so I wouldn't lose sleep over it. Normal cooking amounts of white pepper are very unlikely to matter. Also, alvimopan is usually given only for a short stretch in the hospital after bowel surgery. If you take concentrated pepper or piperine supplements, just mention it to your care team.

What Is Alvimopan Used For?

Alvimopan capsules are used in hospitals to help the digestive tract recover more quickly after surgery that involves removing part of the bowel (partial bowel resection with primary anastomosis — meaning the remaining ends are reconnected). After this type of surgery, the bowel often temporarily stops working, a condition called postoperative ileus.

This medication is only given during a short hospital stay and is not for long-term or home use.
