Green Tea and Alvimopan

Here is the interaction we found between these two, with the evidence behind it.

Moderate

P-GLYCOPROTEIN SUBSTRATES

Why this applies to Alvimopan

Alvimopan falls under P-GLYCOPROTEIN SUBSTRATES — the group Green Tea interacts with.

Green tea might increase the levels and adverse effects of P-glycoprotein (P-gp) substrates.
In vitro research and case reports suggest that green tea inhibits drug efflux by P-gp, potentially increasing serum levels of P-gp substrates. Case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking green tea and certain P-gp substrates.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Alvimopan (Entereg): your body uses a tiny pump called P-glycoprotein (P-gp) to push this drug back out of your gut and limit how much gets into your system. Green tea may slow that pump down a bit, which could let a little more alvimopan build up than usual. In theory, that means slightly more chance of side effects like stomach upset, indigestion, or low potassium.

Honestly, the evidence here is thin. It comes from lab studies and a few scattered case reports, not solid human trials. On top of that, alvimopan is usually given only in the hospital for a short stretch right after bowel surgery, so a casual cup of green tea is unlikely to cause trouble. Still, mention any green tea supplements to your care team so they can keep an eye on things.

About Green Tea

What is Green Tea?

Green Tea
  • Antioxidant support
  • Weight loss
  • Heart health
  • Mental alertness
  • Cancer risk reduction

Green tea is a popular beverage rich in antioxidants called catechins, and drinking it in normal amounts is considered safe for most people. Concentrated green tea extracts are a different story—they may help modestly with weight or heart health for some, but high doses have been linked to rare liver problems. Talk to your pharmacist or doctor before taking green tea supplements, especially if you have liver concerns or are pregnant.

About Alvimopan

What Is Alvimopan Used For?

Brand name: Entereg

Alvimopan capsules are used in hospitals to help the digestive tract recover more quickly after surgery that involves removing part of the bowel (partial bowel resection with primary anastomosis — meaning the remaining ends are reconnected). After this type of surgery, the bowel often temporarily stops working, a condition called postoperative ileus.

This medication is only given during a short hospital stay and is not for long-term or home use.
