One documented case involved someone taking 100 mg daily for 2–4 years who developed irreversible Parkinson disease; another involved a teenager who developed severe neurological symptoms and liver damage from manganese buildup and required intensive treatment including plasma exchange. This 20 mg dose is lower than those extreme cases, but the safety data advises against taking extra high-dose manganese supplements unless a clinician has recommended it.

Theoretically, the risk for manganese toxicity might increase when taken with antipsychotic drugs.
Hallucinations and behavioral changes have been reported in a patient with liver disease who was taking haloperidol and manganese. Researchers speculate that taking manganese along with haloperidol, phenothiazine-derivatives, or other antipsychotic medications might increase the risk of manganese toxicity in some patients.
