HelloPharmacist Scorecard of Herbal Power Flush by BiOptimizers
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
Herbal Power Flush contains nine ingredients total: six are digestive enzymes (amylase, protease, lipase, cellulase, phytase, and psyllium), plus three herbal components (kelp, red clover, and cascara sagrada). The enzymes are meant to aid digestion and nutrient breakdown.
Kelp is a sea plant high in iodine; red clover is an herb containing compounds called isoflavones; cascara sagrada is a plant-derived stimulant laxative. The product also contains hypromellose and water as inactive ingredients.
Does it work?
Leans against
The evidence on these ingredients is limited. For the enzymes—protease, lipase, and phytase—the data we hold shows insufficient reliable evidence to rate their effectiveness for conditions like inflammatory bowel disease and indigestion.
Cascara sagrada is rated possibly ineffective for bowel preparation and has insufficient evidence for constipation relief. Red clover and kelp also lack established evidence in our data for their claimed uses.
In short, we don't have strong evidence that this product delivers on its marketed benefits.
How safe is it?
Well-documented data
Protease, lipase, and phytase are generally well tolerated, though digestive upset and rare allergic reactions have been reported with proteolytic enzymes. Lipase carries a rare risk of serious gastrointestinal effects (like necrotizing enterocolitis) in premature infants.
Phytase data in humans are limited, but occupational exposure to airborne phytase has triggered allergic asthma and hypersensitivity pneumonitis in some workers. Kelp is generally used in foods but its iodine content and possible heavy metal contamination require caution.
Red clover is well tolerated short-term but may cause muscle aches, nausea, or vaginal spotting. Cascara sagrada, when used long-term, can cause low potassium, loss of muscle mass, and rare liver damage; fresh or poorly aged cascara can trigger severe vomiting.
Meds to double-check
Moderate interaction found
Check your medications before taking this product if you use thyroid hormone or antithyroid drugs, blood thinners like warfarin, amiodarone (a heart rhythm medication), estrogen therapy, methotrexate, tamoxifen, corticosteroids, diuretics (water pills), digoxin, or any drugs your liver metabolizes heavily—red clover and cascara sagrada interact with several of these categories. The enzyme ingredients—protease, lipase, and phytase—have no documented interactions in our data, and we could not check amylase, cellulase, or psyllium.
The bottom line
Scorecard at a glanceFully disclosed formula with graded evidence leaning against its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
This is a multi-ingredient digestive enzyme and herbal blend with limited evidence of effectiveness and real interaction risks—especially with thyroid medications, blood thinners, heart drugs, and potassium-affecting medications. If you take any prescription drugs, check them against our interaction tool before starting.
Talk to your pharmacist about whether the specific ingredients suit your situation, particularly if you have thyroid disease or take warfarin.

Herbal Power Flush by BiOptimizers Drug Interactions
HelloPharmacist Interaction Report
Herbal Power Flush by BiOptimizers contains nine ingredients, and we've found documented interactions with 969 individual medications across the product.
The most serious concern is cascara sagrada, which carries multiple Moderate-severity interactions. It may increase the risk of low potassium (hypokalemia) when combined with water pills (diuretics) or steroids, and can raise bleeding risk if you're on warfarin, a blood thinner.
It may also increase digoxin toxicity by lowering potassium levels.
Read the full breakdown — every affected drug type, severity by severity
Kelp (Ascophyllum nodosum) poses Moderate interactions with thyroid medications and amiodarone because of its iodine content—it can alter how thyroid hormone and antithyroid drugs work, and may raise iodine to unsafe levels alongside amiodarone. Red clover carries Moderate interactions with estrogens, methotrexate, and tamoxifen, plus Minor interactions with several drug-metabolizing enzymes (CYP1A2, CYP2C9, CYP2C19, CYP3A4 substrates) and blood thinners.
The enzyme ingredients—protease, lipase, and phytase—have no documented interactions in our data.
We could not check amylase, cellulase, or psyllium against medication databases. Before you start, run your exact prescriptions through the interaction checker on this page—especially if you take thyroid drugs, blood thinners, steroids, water pills, digoxin, or drugs metabolized by your liver.

For each ingredient in Herbal Power Flush with known interactions, here are the types of medications they can affect. Open any type for the detail — or search your exact drug in the checker above.
Red Clover
Estrogens
Theoretically, concomitant use of large amounts of red clover might interfere with estrogen therapy.
Red clover contains phytoestrogens which might have estrogenic activity in some people. Theoretically, red clover might compete for estrogen receptors and interfere with estrogen-containing drug therapy.
Methotrexate (Trexall, Others)
Theoretically, red clover might increase the risk of methotrexate toxicity.
In a case report, a 52-year-old female receiving weekly methotrexate injections for psoriasis developed symptoms of methotrexate toxicity, including severe vomiting and epigastric pain, after three days of taking red clover 430 mg daily. Toxicity resolved after red clover was discontinued. However, no liver function tests or methotrexate levels were reported.
Tamoxifen (Nolvadex)
Theoretically, the phytoestrogens in red clover might interfere with tamoxifen.
In vitro and animal research suggests that genistein, a constituent of red clover, might antagonize the antitumor effects of tamoxifen. However, there is some evidence from an animal study that red clover does not reduce the efficacy of tamoxifen. Until more is known, tell patients taking tamoxifen to avoid red clover.
Anticoagulant/Antiplatelet Drugs
Although some laboratory research suggests that red clover may have anticoagulant and antiplatelet activity, clinical research has not shown this effect.
In vitro research suggests that genistein in red clover has antiplatelet effects, and historically, red clover was thought to have anticoagulant effects due to its coumarin content. However, some experts state that this is unlikely as most natural coumarins have not been shown to have anticoagulant effects, and their content in red clover is low. Additionally, some clinical research in postmenopausal patients found no effect on coagulation or prothrombin time with the use of red clover flowering tops 378 mg daily for 12 months or red clover isoflavone (Rimostil) 50 mg daily for 2 years.
Caffeine
Theoretically, soy might reduce the clearance of caffeine; however, a small clinical study found no effect.
Red clover contains genistein. Taking genistein 1 gram daily for 14 days seems to inhibit caffeine clearance and metabolism in healthy females. However, this effect does not seem to occur with the lower amounts of genistein found in red clover. A clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, red clover might increase the levels and clinical effects of drugs metabolized by CYP2C19.
In vitro evidence suggests that red clover weakly inhibits CYP2C19. This interaction has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, red clover might increase levels of drugs metabolized by CYP2C9; however, a small clinical study found no effect.
In vitro evidence suggests that red clover might inhibit CYP2C9. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of tolbutamide, a CYP2C9 probe substrate.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.
Cascara Sagrada
Corticosteroids
Theoretically, cascara sagrada might increase the risk of hypokalemia when taken with corticosteroids.
Cascara sagrada has stimulant laxative effects, and long-term use has been associated with hypokalemia.
Digoxin (Lanoxin)
Theoretically, cascara sagrada might cause hypokalemia, potentially increasing the risk of digoxin toxicity.
Cascara sagrada has stimulant laxative effects, and long-term use has been associated with hypokalemia.
Diuretic Drugs
Theoretically, cascara sagrada might increase the risk of hypokalemia when taken with diuretic drugs.
Cascara sagrada has stimulant laxative effects, and long-term use has been associated with hypokalemia.
Stimulant Laxatives
Theoretically, cascara sagrada might have additive adverse effects when taken with stimulant laxatives.
Cascara sagrada has stimulant laxative effects and might compound fluid and electrolyte losses when taken with stimulant laxatives.
Warfarin (Coumadin)
Theoretically, cascara sagrada might increase the risk of bleeding when taken with warfarin.
Cascara sagrada has stimulant laxative effects. In some people, cascara sagrada can cause diarrhea. Diarrhea can increase the effects of warfarin, increase international normalized ratio (INR), and increase the risk of bleeding.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.
Kelp
Amiodarone (Cordarone)
Theoretically, combining Ascophyllum nodosum with amiodarone might cause excessively high iodine levels.
Ascophyllum nodosum contains iodine, although the bioavailability of iodine from Ascophyllum nodosum is lower than that of potassium iodide. Amiodarone contains 37.3% iodine and can increase iodine levels. Concomitant use might increase the risk of having excessive iodine levels and adversely affecting thyroid function. Monitor thyroid function.
Antithyroid Drugs
Due to its iodine content, Ascophyllum nodosum might alter the effects of antithyroid drugs.
Ascophyllum nodosum contains iodine, although the bioavailability of iodine from Ascophyllum nodosum is lower than that of potassium iodide. Iodine in high doses has been reported to cause both hyperthyroidism and hypothyroidism, depending on the individual's past medical history. Taking Ascophyllum nodosum while using antithyroid drugs could alter the effects of the antithyroid drugs.
Thyroid Hormone
Due to its iodine content, Ascophyllum nodosum might alter the effects of thyroid hormone.
Ascophyllum nodosum contains iodine, although the bioavailability of iodine from Ascophyllum nodosum is lower than that of potassium iodide. Iodine in high doses has been reported to cause both hyperthyroidism and hypothyroidism, depending on the individual's past medical history. Taking Ascophyllum nodosum while using thyroid hormone could alter the effects of thyroid hormone.
