Manganese at normal dietary amounts is generally well tolerated. High-dose supplements, however, can be harmful.
The main concern is neurotoxicity—damage to nerve tissue—especially with prolonged high-dose use. Serious cases have included Parkinson-like symptoms and, rarely, liver damage (cirrhosis and fatty liver disease).
One documented case involved taking 100 mg daily for 2–4 years; another affected a teenager whose manganese levels were confirmed by blood test and brain imaging. For lactation, the evidence is mixed: while amounts in normal food and prenatal vitamins are appropriate, the safety data on high-dose supplements during breastfeeding isn't clear enough to rate firmly.

Researchers speculate that taking manganese along with haloperidol, phenothiazine-derivatives, or other antipsychotic medications might increase the risk of manganese toxicity in some patients.
