High doses carry a rare but serious risk: neurotoxicity, including symptoms like Parkinson disease (tremors, rigidity, stiffness). One documented case involved someone taking 100 mg daily for 2 to 4 years and developing irreversible Parkinson disease.
Very high manganese exposure has also been linked to liver damage (cirrhosis and fatty liver), though that's uncommon. For pregnancy and breastfeeding, the safety data on file is conflicting — one entry says lactation is likely safe, while others say possibly unsafe or likely unsafe.

Hallucinations and behavioral changes have been reported in a patient with liver disease who was taking haloperidol and manganese. Researchers speculate that taking manganese along with haloperidol, phenothiazine-derivatives, or other antipsychotic medications might increase the risk of manganese toxicity in some patients.
