This product contains one active ingredient: neem leaf extract in dried form. Neem leaf has been studied for its effects on gingivitis (gum inflammation) and dental health, and also for treating lice and acne.
The liquid also contains alcohol, water, and glycerin as inactive ingredients that make up the formulation.
Does it work?
Moderate evidence
The evidence for neem leaf extract is mixed. It's rated possibly effective for gingivitis (gum inflammation), dental plaque, and lice — meaning some clinical research supports its use, but the evidence isn't ironclad yet.
For acne and tungiasis (a parasitic skin infection), the evidence we hold isn't strong enough to rate it one way or another. If you're considering neem for one of these purposes, talk with your doctor or pharmacist about whether the current evidence matches what you're hoping to treat.
How safe is it?
Well-documented data
Neem leaf extract taken by mouth is generally well tolerated in adults, but high-quality safety studies haven't been done. Serious side effects are rare but have been reported — including cardiac arrest, heart rhythm problems, and, in people with a specific genetic condition (G6PD deficiency), hemolytic anemia and jaundice after drinking large amounts of neem tea daily.
Oral neem oil can cause serious harm in children. Neem should be avoided during pregnancy — the safety data rates it as likely unsafe.
There isn't enough reliable information to use it while breastfeeding, so it's best to avoid it then too. If applied to skin, neem is generally well tolerated, though it may cause contact dermatitis in sensitive individuals.

Neem leaf extract also affects several enzyme systems in your liver and kidneys that clear medications from your body. It may increase the levels of drugs processed by CYP1A2, CYP2C8, CYP2C9, CYP3A4 enzymes, or transported by P-glycoprotein — meaning those drugs could build up to higher-than-intended amounts and cause unwanted effects.
Additionally, neem may reduce how well immunosuppressants (medications that calm your immune system) work.
All of these interactions are rated Moderate in severity. While the enzyme effects haven't been reported in actual patients yet, they've shown up in lab studies, so the risk is real enough to warrant checking.

Antidiabetes Drugs
Neem might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Some clinical research shows that neem can lower blood glucose levels in adults with type 2 diabetes, including those already taking metformin.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.
Cytochrome P450 2C8 (Cyp2C8) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C8 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C8 enzymes. So far, this reaction has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C9 substrates.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.

Immunosuppressants
Theoretically, neem might decrease the effectiveness of immunosuppressants.
Animal research suggests that neem might have immunostimulant effects.
P-Glycoprotein Substrates
Theoretically, neem leaf extract might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
