Theoretically, neem leaf extract might increase the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.

This combination pill pairs amitriptyline (an antidepressant) with perphenazine (an antipsychotic). Your liver uses an enzyme called CYP1A2 as one of the ways it helps clear amitriptyline from your body. In lab dish studies, neem leaf extract seems to slow that enzyme down. If that happened in real life, amitriptyline could build up a bit higher than usual, which might mean more drowsiness, dry mouth, constipation, blurred vision, dizziness on standing, or a faster heartbeat.

Here's the important part: this is only seen in test tubes so far, never reported in actual people, so it's theoretical. I wouldn't lose sleep over it, but since neem can also be harmful if swallowed, please check with your pharmacist or doctor before combining them.

Neither one is a prodrug, so more drug in your system would mean stronger effects and more side effects, things like drowsiness, dry mouth, constipation, blurred vision, dizziness on standing, or feeling unusually groggy. Here's the reassuring part: this is only lab data and has not been seen in actual patients. Still, since these medicines already cause sedation, it's worth running neem past your pharmacist or doctor before starting it.

Here's the gist with this combination pill (amitriptyline plus perphenazine). Lab studies suggest neem leaf extract can slow down a liver enzyme called CYP2C9, which is one of the pathways that helps clear these medicines from your body. In theory, if that enzyme is blocked, the drug could linger a bit longer and its effects could be amplified. For these two, that might mean more drowsiness, dry mouth, constipation, blurry vision, dizziness when standing, or a faster heartbeat.

I want to be honest, though: this is based only on test-tube work, not real people, so it's quite theoretical. Food amounts of neem are unlikely to be an issue, but concentrated supplements are a bigger question mark. Please check with your pharmacist or doctor before combining them, and don't change your dose on your own.
