The Original SAMe 400 mg is a single-ingredient supplement containing S-Adenosyl-Methionine (SAMe) at 400 mg per dose. The active form is delivered as SAMe tosylate disulfate.
The tablet also contains inactive ingredients — cellulose, mannitol, citric acid, stearic acid, silica, sodium starch glycolate, modified cellulose gum, methacrylic acid copolymer, hydroxypropyl methylcellulose, triacetin, titanium dioxide, and yellow ochre — which are binders, fillers, and coating materials.
Does it work?
Strong evidence
SAMe is likely effective for osteoarthritis pain and joint function. It's rated possibly effective for depression and for cholestasis (a liver condition involving bile buildup).
For Alzheimer disease and ADHD, there isn't enough reliable evidence yet to say whether it works.
How safe is it?
Well-documented data
SAMe is generally well tolerated in standard doses, though side effects become more common at higher doses. The most frequent side effects are gastrointestinal — nausea, diarrhea, constipation, stomach upset, and dizziness — along with insomnia, nervousness, and sweating.
In rare cases, users have reported rash, itching, temporary hair loss, and blood in the stool. One important caution: SAMe can trigger or worsen mania in people with bipolar disorder, so it's not recommended for that condition.
Pregnancy ratings show SAMe is possibly safe, but data for lactation isn't on file. If you're pregnant or breastfeeding, talk with your doctor or pharmacist before taking it.

The most serious concern is serotonergic drugs. SAMe itself has serotonin-like effects, so combining it with antidepressants or other serotonergic medications carries a moderate risk of serotonin syndrome — a condition marked by agitation, confusion, rapid heart rate, high blood pressure, and muscle rigidity — as well as other serotonin-related side effects including narrowing of blood vessels in the brain.
Levodopa is the second interaction to watch for. SAMe can chemically modify levodopa in a way that reduces how well it works for Parkinson symptoms, so this is also rated moderate in severity.

S-Adenosyl-Methionine
Levodopa
SAMe might reduce the effectiveness of levodopa.
SAMe methylates levodopa, which might reduce its effectiveness for treating Parkinson disease.
Serotonergic Drugs
Taking SAMe with serotonergic drugs might increase the risk of serotonin syndrome and other serotonergic side effects.
SAMe has serotonergic effects. Theoretically, combining serotonergic drugs with SAMe might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. In one case report, SAMe 100 mg intramuscularly was given daily with clomipramine (Anafranil) 25 mg per day. When the clomipramine dose was increased to 75 mg per day the patient experienced serotonin syndrome about 48-72 hours later, requiring hospitalization.
