Ultra Ingredients & Drug Interactions
by UP4
What is this page for?
First and foremost: checking Ultra against your medications. The heart of this page is the interaction checker and the full interaction report — how this product’s ingredients may interact with prescription and over-the-counter medicines you may be taking.
Around that, we add a pharmacist’s high-level view of the product as a whole — what’s inside, the evidence for its stated use, how transparent the label is, and what safety data exists — so you can see the full picture in one place. It’s educational information from our licensed clinical databases and the clinical staff at HelloPharmacist — not medical advice — and we don’t sell or endorse products. Our editorial policy
Ultra is a dietary supplement by UP4 with 10 active ingredients. HelloPharmacist Scorecard of Ultra by UP4
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Low disclosure
UP4 Ultra contains 10 probiotic ingredients — live beneficial bacteria meant to support gut health. The ones we have full data for are Lactobacillus acidophilus DDS-1, Bifidobacterium longum, Bifidobacterium bifidum, and Bifidobacterium breve, plus Bifidobacterium lactis HN019, Lactobacillus paracasei, Lactobacillus plantarum, Lactobacillus casei, and Lactobacillus rhyamnosus — all part of a proprietary blend.
The capsule also contains inactive ingredients: vegetable cellulose, fructooligosaccharides, vegetable magnesium stearate, and silica. These inactive ingredients are fillers and binders that help hold the capsule together.
Does it work?
Moderate evidence
The evidence for these ingredients is mixed and often limited. Lactobacillus acidophilus shows possibly effective ratings for irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis, though the data aren't strong across the board.
Bifidobacterium bifidum is possibly effective for IBS and respiratory tract infections. For most other uses listed — like allergic rhinitis, cognitive decline, anxiety, and asthma — the evidence is insufficient to know whether these probiotics help.
We don't have effectiveness ratings for the other strains in this product.
How safe is it?
Well-documented data
Overall, these probiotics are generally well tolerated in healthy adults and children. Gastrointestinal side effects — bloating, gas, abdominal discomfort, and diarrhea — have been reported but are uncommon.
Because these are live organisms, there is a small theoretical risk of infection in people with very weak immune systems or serious illness, so those patients should check with their doctor first. Rash and itching have been reported in trials, though it's unclear whether the probiotics themselves were the cause.
For pregnancy, Lactobacillus acidophilus and Bifidobacterium bifidum and longum are rated possibly safe, but Bifidobacterium breve has no pregnancy or lactation rating on file.
Meds to double-check
Moderate interaction found
Before you take UP4 Ultra, double-check your antibiotic medications with your doctor or pharmacist — these four probiotics (Lactobacillus acidophilus, Bifidobacterium longum, Bifidobacterium bifidum, and Bifidobacterium breve) interact with antibiotics in a Moderate-severity way. The interaction isn't a reason to skip either one; it just means timing matters.
Space them several hours apart so the antibiotic doesn't wipe out the probiotics you're trying to take.
The bottom line
Scorecard at a glanceFormula with limited ingredient disclosure with some supporting evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
UP4 Ultra is a multi-strain probiotic for people looking to support gut health, particularly for IBS or antibiotic-related diarrhea. The main thing to watch is separating this from antibiotics so the drugs don't kill off the live bacteria.
If you're seriously ill or have a weakened immune system, talk with your doctor or pharmacist before starting. Check your exact medications with our tool on this page before you begin.

Ultra by UP4 Drug Interactions
HelloPharmacist Interaction Report
UP4 Ultra is a 10-ingredient probiotic capsule, and through its Lactobacillus acidophilus, Bifidobacterium longum, Bifidobacterium bifidum, and Bifidobacterium breve content, it interacts with antibiotics.
All four ingredients carry a Moderate-severity interaction: since these probiotics contain live organisms, taking antibiotics at the same time might kill enough of them to reduce the product's effectiveness. The fix is straightforward — separate when you take them, ideally spacing doses several hours apart.
Read the full breakdown — every affected drug type, severity by severity
Additionally, a few other ingredients in this blend — Bifidobacterium lactis HN019, Lactobacillus paracasei, Lactobacillus plantarum, Lactobacillus casei, and Lactobacillus rhyamnosus — we could not check because we don't hold interaction data for them. That doesn't mean they're free of interactions; it just means the information isn't on file here.
Although the most serious documented interaction is only Moderate in severity, the span of individual medications affected is substantial. Altogether, these interactions span 182 individual medications.
Use the search tool on this page to look up your exact antibiotics and confirm whether they're included.
