Based on the available evidence, the overall risk of a clinically significant Ginger drug interaction appears low for most people. None of ginger's listed interactions are rated major, and even the moderate ones rest on limited, mixed, or largely theoretical evidence.

Interactions deserving the most attention

The concern with the most real-world support involves blood thinners. Case reports link ginger to a raised INR with warfarin and the related drug phenprocoumon, and a small study found ginger plus nifedipine reduced platelet clumping more than either alone, so extra bleeding risk deserves respect even though trial results conflict. Ginger may also lower blood sugar, which could add to the effect of diabetes medications, and a few case reports describe increased toxicity when ginger was combined with certain cancer drugs the body clears through pathways ginger may block.

What the rest of the list means

Ginger's interaction list is long mainly because lab studies suggest it may affect enzymes and transporters the body uses to process many medications, so whole drug classes get flagged at once. Most of these predicted effects have not been confirmed in people, and several have not produced meaningful changes in human studies.

Ginger may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs. However, research is conflicting.
Laboratory research suggests that ginger inhibits thromboxane synthetase and decreases platelet aggregation. However, this has not been demonstrated unequivocally in humans, with mixed results from clinical trials. Conversely, other in vitro research suggests that ginger induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. However, this interaction has not been reported in humans.

Two case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking ginger and cancer medications that are P-gp substrates (trametinib, crizotinib). However, the causality of this interaction is unclear due to the presence of multiple interacting drugs and routes of administration.

Ginger might increase the risk of bleeding with phenprocoumon.
Phenprocoumon, a warfarin-related anticoagulant, might increase the international normalized ratio (INR) when taken with ginger. There is one case report of a 76-year-old woman with a stable INR on phenprocoumon that increased to greater than 10 when she began consuming dried ginger and ginger tea.

Ginger might increase the risk of bleeding with warfarin.
Laboratory research suggests that ginger might inhibit thromboxane synthetase and decrease platelet aggregation. In one case report, ginger increased the INR when taken with phenprocoumon, which has similar pharmacological effects as warfarin. In another case report, ginger increased the INR when taken with a combination of warfarin, hydrochlorothiazide, and acetaminophen. A longitudinal analysis suggests that taking ginger increases the risk of bleeding in patients taking warfarin for at least 4 months. However, research in healthy people suggests that ginger has no effect on INR, or the pharmacokinetics or pharmacodynamics of warfarin. Until more is known, monitor INRs closely in patients taking large amounts of ginger.

Calcium Channel Blockers

Theoretically, taking ginger with calcium channel blockers might increase the risk of hypotension.
Some animal and in vitro research suggests that ginger has hypotensive and calcium channel-blocking effects. Another animal study shows that concomitant administration of ginger and the calcium channel blocker amlodipine leads to greater reductions in blood pressure when compared with amlodipine alone.

Cyclosporine (Neoral, Sandimmune)

Theoretically, when taken prior to cyclosporine, ginger might decrease cyclosporine levels.
In an animal model, ginger juice taken 2 hours prior to cyclosporine administration reduced the maximum concentration and area under the curve of cyclosporine by 51% and 40%, respectively. This effect was not observed when ginger juice and cyclosporine were administered at the same time.

Overview

Ginger is the underground stem, or rhizome, of the plant Zingiber officinale. It belongs to the same plant family as turmeric and cardamom. Ginger is native to Asia and is now grown in warm regions around the world, including India, China, and parts of Africa and the Caribbean.

People have used ginger for thousands of years as both a spice and a folk remedy. It has a warm, spicy, slightly sweet flavor and is used fresh, dried, powdered, or as an oil in cooking and drinks.

As a supplement, ginger comes in capsules, tablets, teas, candies, and liquid extracts. Some products are standardized to contain a certain amount of ginger's active compounds, often listed as 'ginger root extract.'

How it works

Ginger contains many active compounds. The most studied are gingerols and shogaols, which give ginger its spicy taste and are thought to be responsible for many of its effects.

Researchers believe ginger may ease nausea by acting on the digestive tract and on signals in the gut and brain that trigger vomiting. It may also help food move through the stomach. For pain and swelling, ginger compounds appear to lower the activity of certain inflammatory chemicals in the body.

Many of these mechanisms come from laboratory and animal studies. While they help explain how ginger might work, they do not prove that it will produce the same effects in every person.

Safety & precautions

Ginger is widely used as a food and is considered safe for most people in normal food and supplement amounts. Higher doses are more likely to cause mild side effects.

Use extra caution if you have a bleeding disorder or are scheduled for surgery, since ginger may slightly affect blood clotting. People with gallstones should also check with a doctor, because ginger can increase the flow of bile.

For pregnancy, ginger is often used to reduce morning sickness, and moderate amounts are generally considered acceptable. Still, you should talk with your doctor before using ginger supplements while pregnant and avoid very high doses. For breastfeeding, there is not enough reliable information, so it is safest to stick to food amounts and check with your doctor before using supplements.

Side effects

Ginger is usually well tolerated. When side effects happen, they are typically mild and may include heartburn, stomach discomfort, gas, burping, or a burning feeling in the mouth or throat.

Higher doses are more likely to cause digestive upset. Rarely, some people may have an allergic reaction. Stop using ginger and seek medical help if you notice signs such as rash, swelling, or trouble breathing.

Ginger: Reported Adverse Effects

Orally, ginger is generally well tolerated. However, higher doses of 5 grams per day increase the risk of side effects and reduce tolerability. Topically, ginger seems to be well tolerated.

Most Common Adverse Effects:

Orally: Abdominal discomfort, burping, diarrhea, heartburn, and a pepper-like irritant effect in the mouth and throat. However, some of these mild symptoms may be reduced by ingesting encapsulated ginger in place of powdered ginger.

Topically: Dermatitis in sensitive individuals.

Orally, common side effects of ginger include nausea, belching, dry mouth, dry retching, vomiting, burning sensation, oral numbness, abdominal discomfort, heartburn, diarrhea, constipation, or a transient burning or "chilly hot" sensation of the tongue and throat. Orally, Number Ten, a specific product composed of rhubarb, ginger, astragalus, red sage, and turmeric, can increase the incidence of loose stools.

Four cases of small bowel obstruction due to ginger bolus have been reported following the ingestion of raw ginger without sufficient mastication (chewing). In each case, the bolus was removed by enterotomy. Ginger is composed of cellulose and therefore is resistant to digestion. It can absorb water, which may cause it to swell and become lodged in narrow areas of the digestive tract.

In one case report, a 59-year-old Japanese female with multiple allergic sensitivities developed pruritus and then anaphylactic shock after taking an oral ginger-containing herbal supplement for motion sickness (Keimei Gashinsan, Keimeido). The patient had used this supplement previously for over 20 years with no allergic reaction. The authors theorized the development of a cross-reactivity to ginger after the use of an oral supplement containing zedoary and turmeric, which are also in the Zingiberaceae family.

In one clinical trial, some patients reported increased menstrual bleeding while taking a specific ginger extract (Zintoma, Goldaru) 250 mg four times daily orally for 3 days. An "intense" urge to urinate after 30 minutes was reported in two of eight patients given 0.5-1 gram of ginger. However, this effect has not been corroborated elsewhere. Dysuria, flank pain, perineal pain, and urinary stream interruption have been reported in a 43-year-old male who drank ginger tea, containing 2-3 teaspoons of dry ginger, daily over 15 years. The adverse effects persisted for 4 years and were not associated with increases in urinary frequency or urgency. Upon discontinuing ginger, the patient's symptoms began to improve within one week and completely resolved after eight weeks, with no relapses six months later.

Dosing

There is no single official dose of ginger that works for every person or every condition. The right amount can depend on the form you use and the reason you are taking it.

Because supplement strengths vary, the best approach is to follow the directions on the product label and not exceed the recommended amount. If you are using ginger for a specific health problem, such as nausea during pregnancy or chemotherapy, ask your pharmacist or doctor what amount and form is right for you.

Standardized 'ginger root extract' products may list a specific amount per dose, but more is not always better. Start low, watch how you feel, and check with a healthcare professional if you are unsure.

Ginger is considered a first-line nonpharmacological treatment option for nausea in pregnancy by the American College of Obstetrics and Gynecology (ACOG). However, it should not be used long-term or without medical supervision and close monitoring.

Possibly Safe

When used for medicinal purposes. Despite some early reports of adverse effects and one observational study suggesting that taking dried ginger and other herbal supplements during the first 20 weeks of pregnancy marginally increased the chance of stillbirth, most research shows that ginger is unlikely to cause harm to the baby. The risk for major malformations in infants of parents who took ginger when pregnant does not appear to be higher than the baseline rate of 1% to 3%. Also, other research suggests that ginger intake during various trimesters does not significantly affect the risk of spontaneous abortion, congenital malformations, stillbirth, perinatal death, preterm birth, low birth weight, or low Apgar scores. Ginger use has been associated with an increase in non-severe vaginal bleeding, including spotting, after week 17 of pregnancy.
