Why this applies to Alogliptin

Alogliptin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.

Aegeline inhibits cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, aegeline may increase levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use aegeline cautiously or avoid in patients taking these drugs.

Here's the honest picture for alogliptin (Nesina). The concern on file is that aegeline may block a liver enzyme called CYP3A4, which could let drugs cleared by that enzyme build up a bit. But this is based only on lab (test tube) data, never confirmed in people, and it's rated a minor interaction.

On top of that, alogliptin isn't really cleared by CYP3A4 in any meaningful way. Most of it leaves the body unchanged through your kidneys, so this enzyme issue likely doesn't apply much to it. My bigger worry is aegeline itself, since it's been linked to serious liver injury and U.S. regulators warned against it. I'd steer clear of it and check with your pharmacist or doctor before adding any supplement.

Aegeline is a compound originally identified in the bael tree (Aegle marmelos) that gained attention because it was added to some weight-loss and workout supplements. Alogliptin tablets — sold under the brand names Alogliptin and Nesina — are used along with diet and exercise to improve blood sugar control in adults with type 2 diabetes mellitus.

Alogliptin is not used for type 1 diabetes or diabetic ketoacidosis, as it would not be effective in those situations.
