Isavuconazonium Sulfate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.

Aegeline inhibits cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, aegeline may increase levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use aegeline cautiously or avoid in patients taking these drugs.

Here's the gist with Cresemba (isavuconazonium): your body relies partly on an enzyme called CYP3A4 to clear it. In lab dish studies, aegeline appears to slow that same enzyme down. If that were to happen in real life, your antifungal could build up a bit and you might notice more of its usual side effects, things like nausea, headache, low potassium, or liver enzyme changes.

I want to be straight with you, though: this is only seen in test-tube research, never confirmed in people, so it's mostly theoretical. The bigger concern is aegeline itself, since it's been tied to serious liver injury and regulators have warned against it. Cresemba can also stress the liver. I'd honestly steer clear of aegeline and run it by your doctor or pharmacist first.

Aegeline is a compound originally identified in the bael tree (Aegle marmelos) that gained attention because it was added to some weight-loss and workout supplements. It is best known for being linked to serious liver injury in a product called OxyELITE Pro, and U.S. regulators warned against products containing it. There is no good evidence it safely helps with weight loss, and it should generally be avoided.

About Isavuconazonium

What Is Isavuconazonium Used For?

Cresemba (isavuconazonium sulfate) is used to treat two serious, life-threatening fungal infections: invasive aspergillosis and invasive mucormycosis. These infections can spread through the body and are most dangerous in people with weakened immune systems.

Cresemba for injection is approved for adults and children 1 year of age and older. Cresemba capsules are approved for adults and children 6 years of age and older who weigh at least 16 kilograms (about 35 pounds).
