Isavuconazonium Sulfate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the gist for Cresemba (isavuconazonium). Your liver uses an enzyme called CYP3A4 to handle this antifungal. Adrafinil turns into modafinil in your body, and modafinil is thought to gently speed up (induce) that same enzyme. In theory, that could clear your antifungal a little faster and lower its blood levels.

Why that matters: with a serious infection like aspergillosis or mucormycosis, you need steady drug levels to keep the fungus in check. If levels dip too low, the medicine may not work as well, which is a real concern. That said, this is only a theoretical interaction based on how modafinil behaves, and the effect is considered weak. Please don't change anything on your own. Just loop in your doctor or pharmacist so they can watch your treatment closely.

About Isavuconazonium

What Is Isavuconazonium Used For?

Cresemba (isavuconazonium sulfate) is used to treat two serious, life-threatening fungal infections: invasive aspergillosis and invasive mucormycosis. These infections can spread through the body and are most dangerous in people with weakened immune systems.

Cresemba for injection is approved for adults and children 1 year of age and older. Cresemba capsules are approved for adults and children 6 years of age and older who weigh at least 16 kilograms (about 35 pounds).
