Why this applies to Nirogacestat

Nirogacestat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.

Aegeline inhibits cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, aegeline may increase levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use aegeline cautiously or avoid in patients taking these drugs.

Here's the situation if you're on Nirogacestat (Ogsiveo), the medication used to treat desmoid tumors. Your body relies heavily on an enzyme called CYP3A4 to clear this drug out. Aegeline, in lab (test tube) studies, appears to slow that enzyme down. In theory, that could let nirogacestat build up higher than intended, which may mean more of its typical side effects like diarrhea, nausea, mouth sores, rash, or fatigue.

I want to be honest about the evidence here: this is only laboratory data, and it hasn't been shown to happen in real people. That said, aegeline has been tied to serious liver injury and regulators warned against it, so I wouldn't recommend it regardless. Please loop in your oncology team or pharmacist before combining anything with nirogacestat.

Aegeline is a compound originally identified in the bael tree (Aegle marmelos) that gained attention because it was added to some weight-loss and workout supplements. It is best known for being linked to serious liver injury in a product called OxyELITE Pro, and U.S. regulators warned against products containing it. There is no good evidence it safely helps with weight loss, and it should generally be avoided.

Ogsiveo (nirogacestat) is used to treat adults with desmoid tumors that are growing or progressing and require systemic (whole-body) treatment. Desmoid tumors are rare, locally aggressive growths that arise from connective tissue.

This indication is specific to Ogsiveo tablets taken by mouth. It is not approved for use in children, as safety and effectiveness have not been established in pediatric patients.
