Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Polatuzumab Vedotin-Piiq
Polatuzumab Vedotin-Piiq falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version for Polivy. The active part of this antibody-drug conjugate (a piece called MMAE) is broken down partly by an enzyme called CYP3A4. Aegeline has been shown to slow that same enzyme down, but only in lab dish studies, not in real people. In theory, if aegeline blocked CYP3A4, the toxic payload from Polivy could clear more slowly and build up, which might mean more of its usual side effects like nerve tingling or numbness in the hands and feet, low blood counts, infections, fatigue, or nausea.
The evidence here is very weak and this has never actually been seen in humans. That said, aegeline itself has been tied to serious liver injury and isn't something I'd recommend regardless. Please loop in your oncology team or pharmacist before mixing anything new with chemotherapy.

About Aegeline
What is Aegeline?
- Weight loss
- Fat burning / metabolism support
- Sports and bodybuilding supplements
- General 'energy' products
Aegeline is a compound originally identified in the bael tree (Aegle marmelos) that gained attention because it was added to some weight-loss and workout supplements. It is best known for being linked to serious liver injury in a product called OxyELITE Pro, and U.S. regulators warned against products containing it. There is no good evidence it safely helps with weight loss, and it should generally be avoided.
