Isoniazid, Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Alkanna interacts with.

Hepatotoxic pyrrolizidine alkaloids (PAs) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the concern with combining Alkanna and Rifamate. Alkanna root contains pyrrolizidine alkaloids, which are compounds your liver can turn into harmful byproducts. That conversion happens partly through an enzyme called CYP3A4. Rifampin is a strong booster (inducer) of CYP3A4, so in theory it could speed up how fast those alkaloids get changed into liver-damaging forms. On top of that, both rifampin and isoniazid can already be tough on the liver on their own.

I want to be honest that this is a theoretical worry based on how these things work, not proven harm in people. Still, stacking a possibly liver-toxic herb on top of two liver-stressing medicines isn't worth the gamble. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please talk with your pharmacist or doctor before using Alkanna in any form.

Alkanna is a Mediterranean plant whose red root has long been used as a dye and as a folk skin remedy for wounds and burns.
