Hydrochlorothiazide, Propranolol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ashitaba interacts with.

In vitro research shows that ashitaba extract inhibits cytochrome P450 (CYP) 1A2. Theoretically, concomitant use of ashitaba with CYP1A2 substrates might decrease the clearance of these substrates and increase the risk for adverse effects. However, this interaction has yet to be reported in humans. Until more is known, use with caution.

This combo pill pairs a water pill (hydrochlorothiazide) with propranolol, a beta blocker that slows your heart and lowers blood pressure. The part that matters here is the propranolol, since your liver partly clears it using an enzyme called CYP1A2. In test-tube studies, ashitaba seems to slow that same enzyme down. In theory, that could let propranolol build up a bit, which might mean a slower heart rate, more tiredness or dizziness, or lower blood pressure than usual.

I want to be honest: this is only a lab-based idea and has never actually been reported in people. Eating ashitaba as a food is unlikely to be a problem, but concentrated supplements are less predictable. Before adding it, run it by your pharmacist or doctor so they can keep an eye on your blood pressure and pulse.
