Brentuximab Vedotin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Here's the gist with Adcetris (brentuximab vedotin). The part of this chemo drug that does the work, a piece called MMAE, gets cleared by a liver enzyme known as CYP3A4. Atractylodes might slow that same enzyme down, which in theory could let more of the drug linger in your body. If that happened, you could see stronger side effects like nerve tingling or numbness in the hands and feet, low blood counts, fatigue, or nausea.

I want to be honest about how thin this evidence is. The only signal comes from animal and lab studies using high doses, and it has never been shown in people. So it's more of a theoretical caution than a proven problem. Still, because you're on serious chemo, please run any supplement by your oncology team before adding it.

What Is Brentuximab Vedotin Injection Used For?

Brand name: Adcetris

Adcetris (brentuximab vedotin) is used to treat classical Hodgkin lymphoma (cHL) in adults — including newly diagnosed advanced-stage disease, as consolidation therapy after a stem cell transplant, and in relapsed or refractory cases. It is also approved for pediatric patients aged 2 and older with previously untreated high-risk classical Hodgkin lymphoma, given in combination with chemotherapy.

Adcetris is also used for several T-cell lymphomas — including systemic anaplastic large cell lymphoma (sALCL), peripheral T-cell lymphomas (PTCL), primary cutaneous anaplastic large cell lymphoma (pcALCL), and CD30-expressing mycosis fungoides (MF). Additionally, Adcetris is used in combination with lenalidomide and a rituximab product for adults with relapsed or refractory large B-cell lymphoma (LBCL) who are not candidates for a stem cell transplant or CAR T-cell therapy.
