Why this applies to Brexpiprazole

Brexpiprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Here's the basic idea with brexpiprazole (Rexulti). Your liver uses an enzyme called CYP3A4 to help clear this medication from your body. In animal studies, high doses of atractylodes seemed to slow a similar enzyme down. If that were to happen in people, brexpiprazole could build up a bit more than usual, which might mean more of its typical side effects like drowsiness, restlessness, dizziness, or weight changes.

The good news is this is mostly theory. It has only been seen in lab and animal research, never confirmed in humans, so the real-world risk looks small. Still, since brexpiprazole affects the brain, I'd mention any atractylodes-containing herbal product to your pharmacist or doctor before combining them, just to be safe.

Atractylodes is a root used for centuries in traditional Chinese, Japanese, and Thai medicine, mostly for digestive complaints and fatigue, often as part of multi-herb formulas. Modern research is still early and mostly from lab and animal studies, so its true benefits in people are not well proven. If you want to try it, talk with a pharmacist or doctor first, and avoid it during pregnancy and breastfeeding.

About Brexpiprazole

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
