Why this applies to Busulfan

Busulfan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Here's the idea behind this pairing. Your body uses an enzyme called CYP3A4 to help clear busulfan. In animal studies (not people), high doses of atractylodes slowed down a similar enzyme. If that same slowdown happened in humans, busulfan could build up a bit higher than expected, which might mean stronger effects like more nausea, mouth sores, or dips in your blood counts.

That said, this is theoretical and only seen in lab and animal work, so I wouldn't lose sleep over it. But busulfan is powerful chemo with a narrow safety margin, so please don't add atractylodes without first checking with your oncology team or pharmacist.

What Is Busulfan Used For?

Brand names: Myleran, Busulfex

Busulfan is used to treat chronic myelogenous leukemia (CML), a cancer of the blood and bone marrow. The specific use depends on the form and product.

Myleran (oral tablet) is used for the palliative treatment of CML — meaning it helps control the disease and relieve symptoms. The intravenous injection forms (Busulfex and Busulfan Injection) are used in combination with another chemotherapy drug, cyclophosphamide, as a conditioning regimen to prepare the body before an allogeneic hematopoietic progenitor cell transplant (a bone marrow or stem cell transplant from a donor) for CML.
