Dolutegravir, Rilpivirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Here's the situation with Juluca (dolutegravir plus rilpivirine) and atractylodes. The rilpivirine half of this combo is partly handled by a liver enzyme called CYP3A4. In lab and animal studies (not in people), atractylodes has shown some ability to slow down this enzyme. If that held true in humans, it could let rilpivirine build up a bit more than usual, which in theory might mean more side effects like headache, trouble sleeping, odd dreams, or mood changes.

The honest takeaway: this is a theoretical, weak concern based only on animal data, and it hasn't been confirmed in humans. Since Juluca is HIV therapy where steady levels matter, I'd still loop in your pharmacist or doctor before adding atractylodes, just to be safe.
