Drospirenone, Estetrol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might decrease the levels of CYP1A2 substrates.
In animals, atractylodes administered at high doses has been shown to induce CYP1A2 activity. This effect has not been shown in humans.

Here's the gist with Nextstellis (the drospirenone and estetrol birth control pill) and atractylodes. The idea is that atractylodes might speed up an enzyme called CYP1A2, which helps process the hormones in your pill. If a supplement revs that enzyme up, your body could clear the hormones a little faster, which in theory could make the pill slightly less effective.

Now, the honest part: this is only seen in animals given very high doses, and it has not been shown in people. So this is theoretical and rated minor. I wouldn't lose sleep over it, but since this pill prevents pregnancy, I'd run it by your pharmacist or doctor before adding atractylodes, just to be safe.

Drospirenone, Estetrol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Nextstellis combines drospirenone (a progestin) with estetrol (an estrogen), and both rely on the liver enzyme CYP3A4 to be cleared from your body. The thinking here is that atractylodes might slow that enzyme down a little, which could let these hormones build up slightly. If that happened, you might notice more of the usual side effects, like breast tenderness, nausea, headaches, mood changes, or spotting.

Honestly though, this is a very theoretical concern. The only signal comes from animal cells given high doses of atractylodes, and nothing similar has been shown in real people. It is graded as a minor interaction for good reason. If you want to use both, just mention it to your pharmacist or doctor so they can keep an eye on things.
