Sorafenib falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Atractylodes interacts with.
Laboratory research suggests that atractylenolides II and III, constituents of atractylodes, reduce platelet activation. So far, this has not been shown in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know with Sorafenib (Nexavar): this cancer drug already carries a real bleeding risk on its own, since it can affect how your blood clots and how blood vessels behave. The concern with atractylodes is that some of its compounds (atractylenolides) appeared to make platelets, the tiny cells that help you clot, a little less sticky in lab dishes.
Put those two together and the worry is extra bleeding or bruising, like nosebleeds, bleeding gums, or cuts that are slow to stop. I'll be honest though, this is only theoretical right now and hasn't been seen in real people. Still, because you're on a serious medication, please run any atractylodes product by your oncologist or pharmacist before starting it.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Sorafenib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Nexavar (sorafenib): your liver uses an enzyme called CYP3A4 to help clear this medicine. In lab and animal studies, high doses of atractylodes seemed to slow down a similar enzyme. If that were to happen in people, sorafenib could build up a bit, which in theory might mean more of its usual side effects, like fatigue, diarrhea, high blood pressure, or that tender, peeling skin on the hands and feet.
I want to be honest, though, this is only a theoretical concern. It has not been shown in humans, and the evidence is weak. Since sorafenib is a serious cancer drug, please don't add any herbal supplement without first checking with your oncologist or pharmacist.

What Is Sorafenib Used For?
Brand name: Nexavar
Sorafenib tablets are approved to treat three types of cancer. The first is unresectable hepatocellular carcinoma (HCC) — liver cancer that cannot be removed with surgery. The second is advanced renal cell carcinoma (RCC) — kidney cancer that has spread or progressed. Both the brand Nexavar and generic sorafenib tablets are approved for these two uses.
The third use is for locally recurrent or metastatic, progressive differentiated thyroid carcinoma (DTC) — thyroid cancer that has spread or worsened and no longer responds to radioactive iodine treatment. Nexavar and sorafenib tablets are both approved for this use as well.
