Acetaminophen, Chlorpheniramine, Dextromethorphan, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Beta-methylphenethylamine (bmpea) interacts with.
In vitro research shows that BMPEA strongly inhibits CYP2D6.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination cold medicine includes dextromethorphan (the cough suppressant), which your body clears using a liver enzyme called CYP2D6. Lab studies suggest BMPEA can strongly block that same enzyme. In theory, that means dextromethorphan could build up in your system, leading to more drowsiness, dizziness, confusion, or in rare cases agitation. Keep in mind this is only a theoretical concern based on test-tube data, not real patient cases.
I'd also point out a bigger picture issue: BMPEA acts like a stimulant, and the pseudoephedrine in this product is also a stimulant. Stacking the two could push your heart rate and blood pressure up more than you'd want. BMPEA isn't a well-studied or approved ingredient, so I'd steer clear of it. Please check with your pharmacist before mixing these.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that BMPEA inhibits CYP3A4 by up to 37%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing about this combo: BMPEA is a stimulant that, in lab studies only, slowed down CYP3A4 (an enzyme your liver uses to clear certain drugs) by about a third. In your cold medicine, dextromethorphan (the cough suppressant) leans on this pathway, so in theory it could build up a bit and lead to dizziness, drowsiness, or feeling foggy. This is theoretical only, based on test-tube data, not real people, so I wouldn't lose sleep over it.
My bigger concern is different. Both BMPEA and the pseudoephedrine in this product are stimulants that raise heart rate and blood pressure. Stacking them can leave you jittery, racing, or with a pounding heart. Honestly, BMPEA is best avoided altogether. Please check with your pharmacist before mixing these.

STIMULANT DRUGS

BMPEA is thought to have stimulant effects, including cardiovascular effects such as hypertension, tachycardia, and cardiac arrest.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
The piece of this cold medicine I want to flag is the pseudoephedrine, which is itself a decongestant stimulant. It works by tightening blood vessels, which can nudge up your heart rate and blood pressure. BMPEA is built much like amphetamine and is also thought to act as a stimulant. Stacking two stimulants together can add up, so the worry here is more pounding heartbeat, a faster pulse, jitteriness, trouble sleeping, and higher blood pressure.
To be honest, this concern is theoretical, based on how these compounds behave rather than solid human studies. Still, BMPEA isn't an approved ingredient and has real heart-safety question marks, so I'd lean cautious. Please check with your pharmacist or doctor before combining them.
