Acetaminophen, Dextromethorphan, Guaifenesin, Phenylephrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Beta-methylphenethylamine (bmpea) interacts with.
In vitro research shows that BMPEA strongly inhibits CYP2D6.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the piece worth knowing in this cold-and-cough combo: the dextromethorphan part is cleared from your body by an enzyme called CYP2D6. Lab studies suggest BMPEA can strongly block that enzyme. If that happens, dextromethorphan can pile up rather than break down normally, which may lead to more drowsiness, dizziness, confusion, or in heavier cases jitteriness and a racing feeling.
On top of that, BMPEA acts like a stimulant similar to amphetamine, and phenylephrine already nudges blood pressure and heart rate up, so stacking them isn't ideal. This concern is mostly theoretical, based on lab work rather than people, so don't panic. Still, before you mix the two, please check with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that BMPEA inhibits CYP3A4 by up to 37%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your cold and cough combo (Conar-A). One of its ingredients, dextromethorphan, is partly cleared by a liver enzyme called CYP3A4. Lab testing suggests BMPEA can block that enzyme a bit, so in theory more dextromethorphan could linger in your body. That might mean stronger side effects like dizziness, drowsiness, or feeling restless or foggy.
I'd be honest though: this is only a theoretical concern based on test-tube data, not real people, so the actual risk is likely small. My bigger worry is the combo itself. BMPEA is an unapproved amphetamine-like stimulant, and phenylephrine in this product also raises heart rate and blood pressure. Stacking the two could push your heart and pressure higher. Please check with your pharmacist or doctor before mixing them.

BMPEA is thought to have stimulant effects, including cardiovascular effects such as hypertension, tachycardia, and cardiac arrest.

Here's the piece to pay attention to in this combo product: the phenylephrine. That ingredient is a decongestant that works by tightening blood vessels, which can nudge your blood pressure up and quicken your pulse. BMPEA is an amphetamine-like stimulant that does much the same thing. Stack the two and, at least in theory, you could see an additive bump in blood pressure and heart rate, plus jitteriness, headache, or palpitations.
To be fair, this is a theoretical concern based on how these compounds behave, not on solid human studies. Still, BMPEA itself is an unapproved supplement ingredient with real heart-related warnings attached. I'd skip it and run any questions by your pharmacist before mixing.

Beta-methylphenethylamine (BMPEA) is a synthetic stimulant chemically similar to amphetamine that has been found in some weight-loss and sports supplements, often without being clearly listed on the label.
